Our Geriatric Medicine physicians provide older adults with specialized care geared towards their needs. The Assessment Clinic, located on the first floor of the Landon Center on Aging in the Geriatric Medicine Suite, aims to maintain older adults in the community at the most independent level possible. This is done through an interdisciplinary approach to provide a comprehensive geriatric assessment.

The clinic's health care team thoroughly evaluates the health, medical condition and functional ability of older adults. In addition to gathering information from the patient's primary care physician, a thorough examination and interviews with both patient and family are used to assess the patient's needs. Personalized recommendations are provided to patients, families, caregivers, and primary care physicians as a result of the consultative office visit, with referrals made to a variety of community resources and service agencies to assist in the long-term assistance to older adults in the Greater Kansas City area.

To schedule an assessment or receive more information, contact the clinic at 913-588-1204.
